Back to School Health and Safety Checklist

Talk to your School Nurse

Let your school nurse know if your child has a chronic health condition like asthma, diabetes, allergies, seizures, etc. The nurse will guide you on what is needed to support your child at school.

  • Bring in the "Health Record Inquiry/Annual Update" form to the school or complete the medical history section of Aeries, providing as much detailed information as possible
  • If your student requires prescription medication, over the counter medication, or a specialized procedure (G-tube, Tracheostomy, etc.) while at school, please consult with the school nurse and see the medication information tab on our website

Reminder: The school must have a physician’s order on file for students to carry and/or administer medication(s) and for specialized health procedure(s) while at school.

Pediatric Tuberculosis (TB) Risk Assessment for New Students

Los Angeles County requires that all students entering a Los Angeles County school for the first time, regardless of grade level, complete a Pediatric Tuberculosis (TB) Risk Assessment one year before enrolling into school up to 3 months after enrolling in school. 

Please have your child’s healthcare provider complete the Los Angeles County Pediatric TB Risk Assessment and submit it to your child’s school.

Oral Health Assessment

California law requires an oral health assessment by May 31 of your child's transitional kindergarten or kindergarten year or first year in public school. Exams completed within the 12 months before your child enters school are acceptable. 

  • Schedule a visit to the Dentist 
  • Bring the signed "Oral Health Assessment Form" to the school

 

Reminder: C-VUSD strives to provide free dental screenings for students throughout the school year. Please stay tuned for updates and communications regarding upcoming free dental screenings at school.

Health Examination for School Entry

A comprehensive physical examination and health assessment is recommended 18 months before first grade up to 3 months after enrolling in first grade.

  •  Take your child to get a physical exam
  •  Bring the signed "Report of Health Examination" Form to the school

 Reminder: Ask your doctor to check your child's vision and hearing. Bring results to the school.
