If a high school student requires a general work permit during the school year, the student needs to go to the high school he/she is currently attending to obtain it. If the student is currently attending a middle school, the student needs to go to their feeder track high school. In order to work, the student must be at least the age of 14 (except for entertainment, agricultural and domestic work).
The Student Services Department at the Covina-Valley Unified School District Office only issues work permits during summer break.
We are located at 519 E. Badillo St., Covina, CA 91723 in the Student Services Department.
We are open from 7:00 am to 4:30 pm.
When requesting the work permit, the STUDENT must bring the following:
- Completed Statement of Intent to Employ Minor and Request for Work Permit (B1-1) form. Both the parent AND the employer must sign the form!
NOTE: ALL SIGNATURES ON THE WORK PERMIT APPLICATION MUST BE ORIGINAL, INCLUDING THE EMPLOYER'S SIGNATURE. A WORK PERMIT REQUEST WILL BE DENIED IF ANY OF THE SIGNATURES ARE STAMPED AND/OR PHOTOCOPIED
- A current ID card (i.e. school, DMV, passport)
- Proof of birth date (i.e. birth certificate, ID with a birth-date on it, official school record with a birth-date on it)
Work Permits expire five (5) working days after school starts and are renewed yearly until the student turns 18 years old.
