SENIORS - SUBMIT THIS FORM ASAP

Seniors must sign and submit the form linked below before JANUARY 15TH to our registrar, Ms. Jauregui via email at ejauregui@c-vusd.org If you are under 18 years of age, your parent or guardian must also sign the form.

The form verifies how you want your name to appear on your diploma, as well as authorizes the release of transcripts and GPA for the Cal Grant award.
